If we do wudhu and ghusul in washroom in which we urinate and defecation, so can we recite Bismillah before entering in washroom or it’s mandatory to recite in washroom again? If someone forgets to recite it before ghusul and wudu so his/her wudhu/ghusul acceptable?

I get vaginal discharge often so I renew Wudhu after every Salah & ignore the discharge. If I have some urge to visit the toilet but don’t go & if I feel some vaginal discharge come due to clenching the back passage through which stool passes, does that invalidate Wudhu?
