ANSWER:

Yes, you must repent sincerely to Allah while showing deep remorse because what you did was a major sin that would take a person out of the fold of Islam. You must never go back to such a sin ever again. You must also break all communications with that woman as such relationships are prohibited and sinful in Islam. 

Allah is the Most Forgiving, The Most Merciful, depend on Him completely and expect the best from Him. 

years back I was in relationship with a girl everything was going alright But suddenly one day she told me that she doesn’t want to be with me anymore I got frustrated and my friend suggested me to visit witchcraft ! So one day we went to him And I told him about it I was not knowing it’s a major sin because I never read Quran!Can I repent? Will Allah forgive me ?

My question is that, for someone who has committed sins in the past and has repented and now found out that because of their past sin they caught a disease. Can they tell the gynecologist about it to help them diagnose and get treatment or they should keep quiet? As we know in Islam it is forbidden to tell about his sin.
