If the toes do slide slowly during sajdah (because prayer rug is slippery) without which it gets separated from the ground, is the sajdah and thus also the salaat valid?

I have a problem where i’m always doubting whether I’ve missed things in my prayer, things like the dua between the two prostrations. it got the point i’m now doing two extra prostration at the end of nearly every prayer. Is what I am doing wrong and what is the solution to this?
