When should we start and end the month of Ramadan. Should we use Global moon sighting ( meaning when we see the new moon anywhere in world) or local moon sighting as the beginning and end?

I’m a bfeeding mum of a 15months old. He eats regular food too. So I started fasting in Ramadan but stoppd fasting frm 22nd of Ramadan. While fasting I didn’t find it so much hard regarding hunger/thirst but I felt so tired & dizzy in last 3/4 days & noticed significant decrease in milkflow. So I’ve stoppd. I broke 1 out of thirst. Am I sinning for not fasting and do kaffara for breaking?

I noticed while fasting something that felt deep at the back of my throat like a lump so I swallowed it. I know I rinsed my mouth many times after sehri and there was no food. If it was food it must have come up from my stomach. If I tried to get it out with my finger would risk being sick. Do I have to do this fast again, or was this fast valid.
